COMMUNITY HEALTH WORKER:
Duties and Responsibilities
Reporting to:
Social Worker Coordinator- Elko County School District Office of Safe and Respectful Learning Environments.
Program Requirements: An informed consent form must be signed by the students parent or guardian, prior to ongoing services being provided.
Duties:
• In partnership with the attendance office and/or the Elko County School District Police; receive reports of truancy, and excessive absences.
• Monitor and report all instances of bullying to the respective site administrator.
• In concert with existing school district bullying programs, design and present individual or group anti-bullying education tailored to meet the individual needs of the student (s)
• Interact with students in a professional manner.
• Make a visible presence in assigned school.
• Individually or in partnership with the guidance department, and school social workers, conduct and facilitate support groups with students as needed.
• Assist students who struggle with issues of phobias, social issues, and coping skills.
• Assist in the research and development of bio-psych-social assessments of students as requested.
• Provide psycho-social rehabilitation services as requested and in accordance with training and education.
• Identify and refer students and families to appropriate community resources for students in partnership with parents and/or guardians (as applicable).
• Develop, coordinate and facilitate conflict resolution sessions between disputing students.
• Assist individual school site staff as needed.
• Serve as point of contact with community agencies serving the needs of students and their families, unless otherwise directed by the coordinator of social workers or school administrative staff, who may choose to serve as the point of contact.
• Work collaboratively with Communities In Schools, (CIS) to enhance services to students directly related to improving academic performance and functioning.
• Adhere to the provision of mandated reporting requirements.
• Complete training requirements for position.
• Adhere to the Code of Ethics of the National Association of Social Workers (NASW)
• Whenever possible, follow “best practices” guidelines of the School Social Worker Association of America (SSWAA).
• Maintain student confidentiality
• Adhere to confidentiality of social worker unit meetings
• Perform other duties as assigned
